Document Checklist After the First Meeting
The following is a list of the information that can be helpful to your attorney during the process of your divorce or dissolution. Please bring what you have available; additional information can be subsequently submitted or obtained.

Personal Financial Statement
Prepare a brief summary listing your assets, debts and income with estimated amounts, values or balances. You can use our form or create your own.
Bank Statements
If available, bring the most recent statement on each bank account, including checking, savings, CDs or money market accounts.
Retirement Accounts
If available, bring the most recent statement for any retirement account, including 401(k), 403(b), IRA, ROTH IRA, pensions, profit sharing plans or other retirement savings accounts for you or your spouse.
Credit Card Accounts
Bring the last three statements from any credit card in your name or your spouse's.
Employment Information
If employed, bring your most recent pay stub and most recent W-2 and that of your spouse if available. Also provide any employment contracts or employee benefit statements.
Pictures
A picture of your spouse and children and a copy your driver's license.
Tax Returns
Federal and State tax returns or the last five years.
Stocks, Bonds, Mutual Funds
Please bring the most recent statement from any such holdings, including copies of stock certificates if available.
Life Insurance Policies
Copies of any and all life insurance policies you own or are a beneficiary of.
Motor Vehicles
Copy of the title of any motor vehicle you own individually or with any other person, if available.
Court Papers
If the divorce is already filed (or over), bring any papers you have on it.
Domestic Violence
If you have been involved in a domestic violence case, please bring any available case, please bring any available paperwork about it.
Health Insurance
We will need information on all medical coverage and a copy of your health insurance cards.
Real Estate
Please bring any deeds, most recent mortgage statements, tax value assessment, recent appraisals and/or closing statements for any real estate held by you individually or with any person or entity.
